Please see the “Initial Consultation Documents” box to obtain the required documents for your Initial Consultation. There will be 3 document files. Please download and fill out all of the required documents to the best of your ability. When complete, please send them to firstname.lastname@example.org and indicate the type of interaction method you would prefer – email, FaceTime or telephone. Once I am provided with all of the necessary information I will get started on your personalized plan and we will schedule an appointment to review protocol.
Should you have any troubles with the electronic documents provided please inform me immediately so we are able to resolve the issue as soon as possible.
