Please see the “Follow Up Session Document” box to obtain the required document for your Follow Up Session. There will be 1 document file. Please download and fill it out to the best of your ability. When complete, please send it to firstname.lastname@example.org and indicate the type of interaction method you would prefer – email, FaceTime or telephone. Once I am provided with all of the necessary information I will get started on your Follow Up Session and we will schedule an appointment to review protocol.
Should you have any troubles with the electronic document provided please inform me immediately so we are able to resolve the issue as soon as possible.
